If you have perused the internet, watched television or have just been alive for the past two years you’ve probably heard about GLP-1 agonists like semaglutide and tirzepatide. Common brand names are Ozempic, Mounjaro or Zepbound. For the sake of this article I will call them, GLP 1 Agonists.
These powerhouse medications, originally developed for diabetes management, have been making waves for their impressive ability to promote weight loss, reduce inflammation, and improve overall metabolic health. But what exactly are they, and why should you care? Let’s break it down.
The Science Behind GLP-1 Agonists
GLP-1 (Glucagon-Like Peptide-1) is a hormone that plays a critical role in regulating blood sugar, appetite, and digestion. Semaglutide (commonly known under brand names like Ozempic and Wegovy) and tirzepatide (branded as Mounjaro and Zepbound) mimic this hormone, helping to control hunger and improve insulin function. Compounded versions of these medications, prepared by specialized pharmacies, offer an alternative option for those seeking weight loss and metabolic improvements. Often these compounded versions have an added ingredient designed to improve palatability of the medication.
The GOOD: More Than Just Weight Loss
While the biggest buzz around GLP-1 agonists is their ability to shed excess pounds, the benefits go way beyond just looking good in your jeans. Here’s what you can expect:
- Weight Loss – By reducing hunger and slowing digestion, these medications help create a natural calorie deficit, leading to sustainable weight loss.
- Lower Inflammation – Chronic inflammation is linked to conditions like heart disease and autoimmune disorders. GLP-1 medications have been shown to reduce inflammatory markers in the body.
- Improved Cholesterol – Many patients see a decrease in LDL (“bad” cholesterol) and triglycerides, which supports heart health.
- Lower Stroke & Heart Attack Risk – With improved metabolism and cholesterol levels, the risk of cardiovascular events is significantly reduced.
- Diabetes Prevention – If you’re pre-diabetic, these medications can help stabilize blood sugar levels and may prevent progression full-blown diabetes.
GLP-1 Agonists: The BAD; What You NEED to Know
One critical factor is the risk of vomiting while under anesthesia. I know, because I have experienced this first hand.
These medications are effective and popular. As a nurse anesthesiologist, I can guarantee on any given day, I will encounter at least one patient taking this medication whether for diabetes or weight loss purposes. GLP-1 agonists slow gastric emptying, meaning food stays in your stomach longer than usual. This is one of the reasons, people just don’t feel hungry while taking these medications.
If you are scheduled for surgery or a procedure requiring anesthesia, it is extremely important to disclose that you are taking this medication. Anesthesia increases the risk of inhaling those excess stomach contents into your lungs as you drift off to sleep. This can lead to severe complications. I always advise my patients to stop taking injectable semaglutide or tirzepatide at least 7 days prior to their scheduled procedure.
Managing Side Effects: Yes, You Can!
Like any medication, GLP-1 agonists come with side effects. The most common ones include:
- Nausea
- Vomiting
- Constipation
The good news? These can often be managed with lifestyle and diet modifications:
- Eat smaller, protein-rich meals to keep nausea at bay.
- Stay hydrated and increase fiber intake to combat constipation.
- Avoid greasy, heavy foods that can trigger digestive discomfort.
